Supportive Housing Services

Empathy and Connection for the Journey Home

Alcott’s supportive housing services come alongside community members who are experiencing both homelessness and mental health challenges, and supports their journey back into housing. Case Managers work with clients by providing personalized housing assistance, even driving across the entirety of Los Angeles County on a regular basis to check in on them, idiomatically and physically meeting them where they’re at.

How We Help

Alcott’s supportive housing services are provided through a program known as ICMS (Intensive Case Management Services). For those who are additionally navigating barriers involved with re-entry, Alcott provides specialized services through a complementary program known as ICMS-ODR (ICMS in partnership with the Office of Diversion and Reentry). Finally, within Alcott’s ICMS-ODR program, enhanced support is provided to new and expecting mothers through a Maternal Health program. ICMS

Comprehensive Support Through Intensive Case Management

Alcott’s ICMS program serves individuals experiencing homelessness who are referred by Los Angeles County’s Coordinated Entry System (CES), providing intensive case management, housing navigation, and linkages to mental health services. These services provide consistent, personalized support integrating housing support, mental health care, and access to essential resources. ICMS operates in partnership with the Los Angeles County Department of Health Services’ ‘Homeless Services and Housing’ program.

Regular in-person engagement designed to stabilize clients, maintain ongoing contact, and provide continuous support in real-world settings.

Assistance connecting individuals to permanent housing options, resolving housing barriers, and coordinating services that promote long-term stability.

Over 80% of Alcott clients being assisted with housing services will be moved to permanent housing over the course of a year, and over 85% of clients being supported with justice-involved re-entry services will not return to incarceration.

Linkages to mental health treatment, medical care, benefits programs, and community resources that address multiple areas of need.

Support built around each individual’s goals, needs, and circumstances, ensuring services align with personal priorities and long-term progress.

Coaching that helps clients strengthen routines, build independence, and develop practical skills needed for everyday stability.

ICMS-ODR

Supporting Successful Transition Back into the Community

Alcott’s ICMS-ODR program serves individuals experiencing homelessness who are referred by the Office of Diversion and Re-Entry, supporting those who are returning to the community after justice system involvement, helping them address mental health and housing needs with coordinated, person-centered care. ICMS-ODR works to reduce recidivism rates, providing rehabilitation and necessary treatment for those experiencing mental health challenges as an alternative to jail time.

Maternal Health Program

Alcott’s Maternal Health program helps new or expecting mothers who are navigating re-entry find permanent housing. Program staff meets biweekly with the OBGYN, jail team, and doulas from the MAMA’s Neighborhood (a DHS program that supports with pre and postnatal care for maternal health clients) to assess new referrals.
